You will require a D4 medical if:
You are applying for a Group 2 licence for the first time

If you wear glasses you must bring your recent prescription with you to your appointment. If you do not have a prescription available please contact your optician who will be able to provide this for you.

Appointments typically take 10 to 20 minutes. We aim to make the process smooth, professional, and efficient.

We will complete and return the signed D4 form to you at the end of the appointment. It is your responsibility to submit it to the DVLA along with your licence application.

Cancellations made at least 72 hours in advance are fully refundable. Cancellations made after this are not, however we may be able to reschedule it for you at no extra cost, at our discretion.
